Consent and Payment Form

This form should be used for consent, medical information for trips, payments and other school events. All trips will be offered on a first-come, first-serve basis. Once this electronic form has been completed, it will be sent directly to the school Finance Office for action. Once your electronic form has been received, a copy will be emailed back to you for your records.

For overseas trips, please be aware that we will require a copy of your child's passport and EHIC card before we can confirm your place.
